OUR VISION
Shelton Care Management, Inc. founder Patricia Shelton has first-hand experience with the exhaustion that comes with managing the care needs of aging parents. She was inspired to create a senior care concierge service to support others on the difficult journey to ensure the health and well-being of their aging or disabled loved ones.
Our mission is to meet the ever-changing needs of our seniors, those challenged with disabilities, and their families, by serving their care needs now and planning for the future. We coordinate with home health agencies, the Veterans Administration, a variety of health plans, financial and legal agencies, assisted living facilities, skilled services partners, and specialty providers.
We strive to bring peace of mind to families as we improve our clients’ healthcare outcomes and advocate for their overall well-being.
YOUR TEAM
Your Central California team includes professional, nationally Certified Care Managers (CCMs) and registered nurses (RNs). Our CCMs have the education and experience necessary to coordinate all aspects of care. With experience in a variety of fields – nursing, social work, psychology, gerontology, and social work – we expertly coordinate all care resources for your loved one’s geriatric or special needs.
Your CCM is an aging life care expert who assists clients in maximizing their functional potential and autonomy in a safe and suitable environment. By utilizing local resources and support, your CCM coordinates the most cost-effective care possible to meet the client’s needs and goals. CCMs adhere to nationally reviewed practice standards, a strict code of ethics, and have no conflicts of interest.

Start with a nationally recognized Comprehensive Assessment conducted by a professional Certified Care Manager. This assessment lays the groundwork for identifying practical solutions for your medical, safety, and social concerns using specific local resources.
As a liaison between you and your aging or disabled loved one, your personal Certified Care Manager ensures clear communication between all parties involved in decisions or provision of care: doctors and other healthcare professionals, caregivers, attorneys, fiduciaries, and family members.
You’ll gain peace of mind with competent guidance through the complex healthcare maze. We’ll ensure that your loved one’s comprehensive care plan has consistent monitoring and that agreed-upon goals are being met.
